Let’s start here and see what will happen at your visit to Keszei Vision Care.  Your first step will be to make an appointment.  You can call for an appointment at (269) 459-8900 and speak to our receptionist or use our Request an Appointment listed below.  Once your appointment is set, you can save a lot of time at your visit by filling out most of the paperwork prior to your visit.  Yes, this information is important for your visit and is a necessity.  You can provide this information by entering our patient portal.  The nice feature of using the patient portal is that you won’t need to fill out a ton of forms once you arrive at your visit.  The information goes directly into your chart.  That way, the technician won’t have to re-type all of your information from the registration forms to the computer, saving you time.  You can also download the forms (located here), fill them out and bring them in.

Once you call for an appointment and provide us with an email address, we can send you a sign up and special instructions on how to use the Patient Portal to register your information.

Canceling an Appointment

Things may come up that make it hard to keep your appointment. Please call us with as much advance notice as possible to cancel or change your appointment. This helps us to be able to see other patients who need to see the doctor. We must be notified of a cancellation prior to your appointment or your appointment will be considered a no-show. If you miss an appointment and do not et us know before hand, you will incur a $25.00 fee.  A credit card on-file will be necessary to make any future appointments if you no-show for an appointment.  We may dismiss you from our practice and ask you to find another doctor if you are a no-show for three appointments within a 12-month period. 

If you miss your appointment, please call us as soon as possible during our regular business hours to reschedule. A missed appointment will appear as a no-show unless you notify us prior to your appointment.

For a successful appointment, please bring:

  • Completed paperwork, if you did not fill out on patient portal
  • Insurance Card
  • Photo ID
  • Current medications
  • Medical records and lab results, if any
